Use this secure, tracked process to exercise privacy rights with Modern Doc, Inc. You do not need to create an account.
Choose the action you want, verify control of your email address, and save the reference and private access key in your receipt. We do not ask for health details at intake. If more information is reasonably necessary to authenticate or fulfill the request, we will arrange an appropriate follow-up.
Washington requests are timed from receipt. Nevada requests are timed from authentication. We work to respond without undue delay and show the current deadline in your receipt and protected status view.
